We are looking for a Social Worker with the Palliative Care Department at St. Jude Medical Center.

Location: Fullerton, CA
Schedule: Full Time - 80 Biweekly Hours
Shift: 8-hour, Days

Job Summary:
The Social Worker, under general supervision of the Palliative Care Manager. The Social Worker will work as a part of multi-disciplinary team and will collaborate with team members to provide whole person care.  The Social Worker will initiate psychosocial assessments, provide case management, and support services and counseling to patients and families.  Additionally, the Social Worker will assist the team with advance care planning and goals of care discussions.  The Social Worker will make appropriate inpatient and outpatient community referrals, including referrals to outpatient Palliative Care and/or Hospice services.

  • Competence with common office productivity software and ability to learn web based applications used regularly in the course of work.
  • Own transportation.
  • Demonstrates skills in education, group presentation, and public speaking.
  • Preferred: Asian or Spanish language fluency and relevant cultural competence skills.
  • Preferred: Ability to develop and deliver educational workshops to small and large groups.
  • PALLIATIVE CARE:
    • Knowledge of psychosocial needs of patients and families coping with chronic, serious, life-limiting, and/or end of life diseases. 
    • Knowledge of advanced care planning processes, issues, and tools, ie: POLST, Advanced Health Care Directives. 
    • Knowledge of community resources and benefits for palliative care and experience in case advocacy. 
    • Knowledge of bereavement skills and resources. 
    • Knowledge of MS Office computer applications. 
    • Ability to deliver education, in-services, and workshops to small and large audiences.

Minimum Position Requirements:

Education: Bachelor's Degree in Social Work from an accredited School of Social Work, or in Psychology, Sociology, Gerontology, Human Services or a related field from an accredited university.

Experience:

  • Two (2) years of full time paid case management experience working with older adults or persons with disabilities.
  • Candidates with Masters of Social Work degree from an accredited School of Social Work may apply with no prior paid work experience.

License and Certificate:

  • California driver’s license.
  • Auto insurance.
  • CPR certification
  • Advanced Palliative Hospice Social Worker- Certified (APHSW-C) preferred or Advanced Certified Hospice & Palliative Social Worker (ACHP-SW)

Preferred Position Requirements:

Education: Master's Degree in Social Work from an accredited School of Social Work approved by the Counsel for Social Work Education.

Experience:

  • Minimum 2 years Hospice Experience
  • Minimum 2 years working in an acute hosital setting
  • Social service case management experience working with families and older adults who are coping with adult onset cognitive impairments that include dementia, Parkinson’s disease, Huntington’s disease, Multiple Sclerosis, or related diagnosis.

License and Certificate: Eligible for registration with the California Board of Behavioral Sciences as a candidate for licensure/A.
