Senior Program Manager
We are looking for a Senior Program Manager for the HSS Administration SJHH Department at St. Joseph Heritage Healthcare.
Location: Anaheim, CA
Work Schedule: Full time - 80 Biweekly hours
Shift: 8 hour, Days
Job Summary:
The Senior Program Manager has oversight for all aspects of the Southern California Skilled Nursing Facility (SNF) Program in order to enable long-term and short-term goals to be achieved in a manner that is consistent with the Southern California Region. This position will work closely with the Southern California Leadership Team in developing and maintaining the strategic direction of the SNF Collaborative. The Senior Manager is accountable for achieving established performance targets through actively engaging multiple disciplines and professional staff with both internal and external stakeholders. Is a member of the SNF/AL Leadership Team across all regions. Provides expertise and leadership to support Providence SNF/Acute ministries with key strategies and operational achievements. May be assigned as a licensed Administrator as needed
Essential Functions:
- Works collaboratively with each local ministry to develop Continuum of Care (SNF) Committees, creating regional standard work between the local ministry and Skilled Nursing Facilities. The goal of the committees will be to create standard processes, establish and monitor metrics that address quality, readmissions, utilization, length of stay, communication and an improved patient experience. Best Practices will be shared locally.
- Works collaboratively with leadership of the SNF facilities and providers to improve quality, utilization and patient experience.
- Responsible for the creation, implementation and ongoing measurement of all agreed upon SNF metrics. Responsible for coordinating patient surveys through the current vendor including:
• Data collection.
• Data reporting.
• Revision of survey questions.
• Maintenance of vendor relationships, including contracting, payment for work, etc.
- Works in collaboration with both Legal and Contracting to develop a preferred SNF provider network.
- Works with assigned Skilled Nursing Facilities to assist Home and Community Care entities to capture referrals upon discharge from SNFs.
- Collaborates with all network medical directors to establish standard work for contracted SNF physicians and providers.
- Leads and manages complex projects that are strategic in nature and scope. Creates project summaries and reports, leads project committees/workgroups, and provides presentations and other communications to local and system-wide groups/teams in partnership with division leadership as needed.
- Uses knowledge of quality improvement methodology and change management when performing process improvement and consulting with and directing leaders, managers and caregivers.
- Leadership:
• Participates in various work groups and committees that provide a venue for all stake holders to create new processes, identify and resolve process issues.
• Fosters an environment of accountability.
• Maintains comprehensive knowledge of payor and Regulatory requirements assuring that all functions meet internal, Health Plan, and regulatory body requirements.
- Serves as a member on the Providence System Skilled Nursing Facility (SNF)/Assisted Living Leadership Forum with leader representatives across all regions setting the strategies, initiatives and key performance indicators for SNF performance.
- Provides expertise and input on performance outcomes in budgeting, per patient day labor and expense management, as well as revenue enhancement to assist Administrators and SNF ministry leaders across all regions meet targets.
- Acts as a liaison between Administrators and key leaders of Providence CA SNFs and the SNF Collaborative Network across the Southern California region
- Serves as a member of and acts as a liaison to the LA County Providence Risk Team (Facey, Axminster and Providence Medical Group).
- Acts as a liaison between the Home Care Services Care Coordination Hubs in LA and the associated SNF Collaborative members to ensure the proper partnerships and relationships are in place to ensure that we can navigate between our partner SNFs and Providence Home Health and Hospice.

Minimum Position Qualifications:
Education: Bachelor's Degree in Business Administration or Healthcare related field.
Experience:
- 8 years related experience in SNF operations.
- 6 years leadership experience.
- Strong project management and operations experience.
- 10 or more years experience in a healthcare environment.
Licenses/Certifications:
- Automobile insurance for employee-owned vehicles.
- Valid California driver's license.
Preferred Position Qualifications:
Education: Master's Degree in Business Administration, Health Care Administration, Public Health, or related field.
Experience:
- 1 year Hospital experience.
- 10 or more years experience in a healthcare environment.
Licenses/Certifications: Nursing Home Administrator License
