We are looking for a Hospitalist Nurse RN Coordinator for the HUM Hospitalist Department at St. Joseph Heritage Healthcare.

Location: Eureka, CA 
Work Schedule: Full time - 80 Biweekly hours
Shift: 8 hour, Days

Job Summary:
The SJHMG Hospital Based Services RN Coordinator position provides support to St. Joseph Health Eureka and Redwood Medical Centers Hospitalist and Emergency Department programs. Under the direction of the Hospitalist Program's Medical Director and Program Administrator, the Hospitalist Nurse Coordinator (HNC) remains responsible for the day to day support functions for the Hospitalist team; Serves as a liaison to physicians, staff, discharged patients and their families to facilitate safe and effective flow of patient care in a manner consistent with the Mission, Vision and Values of St. Josephs Heritage Healthcare;  Manages patient assignments to physician teams to optimally meet the patient, hospital unit and Adult Hospitalist to ensure continuity of care and staffing needs; Manages patient/family inquiries and facilitates the completion of appropriate paperwork and documentation after discharge, as needed; Completes and/or Coordinates completion of paperwork requiring physician signature, as appropriate and applicable; Provides support and assistance in quality and organizational improvement activities; Provides coordination and support to expedite efficient and effective patient throughput and flow; and participates in organizational quality improvement activities.

Essential Functions:

  • Serves as a liaison to physicians, staff, discharged patients and their families to facilitate safe and effective flow of patient care in a manner consistent with the Mission, Vision and Values of Providence St. Josephs Health.
  • Manages patient assignments to physician teams to optimally ensure continuity of care and resource utilization.
  • Evaluates, investigates and resolves staffing issues and concerns in collaboration with the Medical Director, Program Administrator and unit managers.
  • Manages patient/family inquiries and facilitates the completion of necessary paperwork and documentation, primarily after discharge.
  • Assumes a leadership role and acts as a resource person to staff, physicians and discharged patients.
  • Provides coordination and support to expedite efficient and effective patient throughput and flow.
  • Acts as a liaison for community providers and agencies to communicate with Hospitalists, Primary Care Physician or Specialist to ensure a continuum of care and coordination of discharge plan.
  • Works collaboratively with primary care physicians, specialists and surgeons to coordinate post discharge patient needs.
  • Uses the nursing process to assess and coordinate an appropriate treatment plan with the Hospitalist, Primary Care Physician or Specialist to follow-up on new diagnosis after hospitalist reviews labs, pathology and imaging reports received after discharge.
  • Triages calls by answering questions from discharged patients and uses the nursing process to assess, plan and implement patient care.
  • In emergency situations, coordinates the scheduling of primary care physician appointments to assure timely follow-up in collaboration with Case Management and Guided Patient Services.
  • Orients new Adult Hospitalists to patient care areas, policies and procedures and Adult Hospitalist workflow.
  • Participates in organizational quality improvement activities. Assists in developing quality improvement activities.  Assist in coordinating data collection and works collaboratively with other departments to interpret data to implement quality improvement activities.
  • Handles highly confidential and sensitive information with discretion according to HIPAA rules and regulations.
  • Assists with special assignments and projects as assigned in areas of program development and metric improvement.
  • Meets with charge nurses, staff, and social workers daily or as needed to review potential problem areas.
  • Regularly updates policies and procedures for SJHMG HBO program.

Skills:

  • Knowledge of The Joint Commission and Core Measures standards in order to conduct chart reviews and provide education to Hospitalists, Primary Care Physician or Specialist on these standards.
  • Ability to manage conflict situations involving physicians and staff. Seeks guidance from in-house experts when needed.
  • Ability to facilitate patient care through the nursing process of assessment, planning and implementation of a treatment plan.
  • Ability to maintain confidentiality in all health system related business.
  • Ability to balance multiple priorities simultaneously, ensuring the timely and accurate completion of each task while maintaining quality standards.
  • Ability to work independently with minimal or no direction.
  • Ability to analyze situations accurately and implement an effective course of action using critical thinking.
  • Demonstrates excellent verbal and written communication skills.
  • Strong interpersonal skills with internal and external contacts
  • Strong customer service orientation.
  • Strong computer and data entry skills.
  • Able to prioritize and process multiple tasks/responsibilities.
  • Excel and Word application knowledge preferred.

Minimum Position Qualifications:

Education: Bachelor's Degree in Nursing.

Experience: 2 years of Hospital experience.

Licenses and Certifications:

  • Current BLS Certification.
  • Valid California RN license.
  • Valid California driver’s license.

Preferred Position Qualifications:

Experience: Hospitalist program experience.
